Arizona State requires that all referral agencies provide their clients with a Business Practice Disclosure, so each client is aware of how we do business and where we receive our fees. They require that the client acknowledge receipt of this disclosure, before beginning services. SENIOR LIVING ADVISORS BUSINESS PRACTICES AND FEE DISCLOSURE

We (Senior Living Advisors) are in the business of referring clients to Independent Living Communities, Assisted Living Facilities and Memory Care Facilities. We will be paid by the facility or home if you move into one of the referred facilities or homes and stay over 30 days. The fee we receive from the facility or home into which you move typically is 50 percent of your first month’s rent excluding additional care charges (Referral Agents do not get paid for Additional Care Charges).

Regarding ALTCS (Arizona Long Term Care System), per Arizona Law, Referral Agencies cannot receive a referral fee from an Assisted Living Facility for an ALTCS approved patient, unless they still have enough funds to pay for their own Assisted Living Care for a minimum of one month. Therefore, we may be able to work with a patient in this situation, but this will be determined during consultation on a case-by-case basis.

CONFLICTS OF INTEREST – Senior Living Advisors holds referral contracts with each of the care facilities we refer our clients to. We do not have common ownership, control of, or any other financial, management or familial relationship with any of the care homes or facilities to which we are referring you.

TERMINATION OF AGREEMENT - By providing us with a written or electronic notice, you have the right to terminate our services at any time, including our use of your personal information. If you terminate our services, we will not be entitled to a fee for any move-in you make after the date of the termination notice UNLESS either….

1. The Assisted Living Facility you choose within the next twelve months is one that we specifically identified and referred to you, before we received your notice of termination.

2. You communicate with us before you move into the facility.
